Case Management

Case management provides additional support services for adults with developmental disabilities. Case management is designed to meet some of the educational, vocational, residential, financial, social, and other identified needs of an individual.

Typical activities include arrangement, linkage, or integration and coordination of multiple services. It also includes assessment and ongoing reassessment of the resident’s needs for services; informing the resident about benefits, community resources, and services; arranging for the receipt of benefits and services; assessment of the resident’s response to treatment and subsequent need to modify the existing array of services being provided; and monitoring the provision of services to the resident, which may include reviewing records and developing and revising the support plan based on the resident's current needs.

RSI works closely with each person to create an individually tailored package, arranged to meet personal and family circumstances.
